Creating Your First Form
In this guide, you'll create a fully branded testimonial collection form, configure its settings, and share your first collection link — in about 3 minutes.
1. Navigate to Forms
From your dashboard, click Formsin the left sidebar. If this is your first time, you'll see an empty state with a "Create form" button.
2. Create the form
Click Create form. A dialog will appear. Enter a name for your form:

- • Customer Reviews
- • Product Feedback
- • After-Service Survey
- • [Your Business Name] Reviews
A URL slug is automatically generated. Click Create form in the dialog to confirm.
3. Configure your form
Click your new form to open its settings. Work through each section:
① Company identity
Enter your business name and a short description. This appears at the top of the form so reviewers know they're reviewing you.
② Pick a template
Select Classic (works on all plans) or a premium template (Starter/Pro). The template sets the visual style of your form.
③ Accent color
Choose a hex color for your submit button. Use your brand primary color to make it feel on-brand.
④ Enable AI assist (optional)
Toggle on AI Writing Assistant to help reviewers write better testimonials. Available on all plans.
Click Save changes when done.
4. Share your form link
Your collection link is displayed at the top of the form settings:
https://reviuly.com/f/your-form-slugClick Copy to copy it to your clipboard. Share it via email, WhatsApp, social media, or link from your website.
5. Get your first testimonial
Open your form link in a new tab and submit a test testimonial yourself. Then go back to your dashboard and navigate to Testimonials— you'll see it with a "Pending" status.
Click Approve and toggle on the Wall and Embed flags to make it visible publicly.
6. Embed on your website
Go to Forms → [Your Form] → Embed and copy the widget code:
